Intake & Paperwork
Before your visit, you'll complete a comprehensive health questionnaire and informed consent through our online portal. This allows us to understand your health history, current concerns, and goals before you walk through the door.
Comprehensive Examination
Dr. Greg will conduct a thorough musculoskeletal and nervous system evaluation. This includes postural analysis, range of motion testing, neurological assessments, and palpation to understand exactly how your body is functioning.
Personalized Care Plan
Based on your examination findings, we'll discuss what we've discovered and create a care plan tailored to your unique needs. We'll explain our approach, answer all your questions, and — if appropriate — begin your first adjustment.

Comprehensive Musculoskeletal & Nervous System Assessment
Our evaluation goes beyond the surface. We assess how your entire body is functioning — from the alignment of your spine to the responsiveness of your nervous system. This comprehensive approach allows us to identify the root cause of your concerns, not just treat symptoms.

Informed Consent
A clear explanation of chiropractic care, what to expect during your visits, and your rights as a patient. Transparency and consent are foundational to our practice.

Insurance & Payment
A Cash-Based Practice
Summit Family Chiropractic is a cash-based practice, which means we keep our focus entirely on you — not on insurance company requirements or limitations. This model allows us to provide the highest quality, most personalized care without the constraints that insurance-driven practices often face.
By working directly with our patients, we can spend more time with you, offer flexible scheduling, and tailor every aspect of your care to your individual needs and goals.
Insurance Reimbursement
We're happy to provide SuperBill summaries after each visit. A SuperBill is a detailed receipt that includes all the diagnostic and procedure codes your insurance company needs to process a reimbursement claim.
Many of our patients successfully submit these to their insurance carriers and receive partial or full reimbursement, depending on their plan's out-of-network benefits. We're happy to help guide you through this process.

Our office is located at 2010 Lincoln Blvd in Venice. There is free 1-hour street parking available on:
- Both sides of Lincoln Blvd
- Down Victoria Ave (south side)
- Venezia Ave

Complete Paperwork Ahead of Time
Filling out your intake forms online before your visit saves time and allows us to review your history in advance for a more productive first session.

We welcome all questions — about chiropractic, Network Spinal, your specific concerns, or anything else. Curiosity is part of the healing process.
Stay Hydrated
